Registered Nurse - Community Hybrid

As our first Registered Nurse, you’ll be at the forefront of shaping our clinical services and making a significant impact within our homecare team. This autonomous position offers a unique chance to help establish and lead our clinical services, providing essential Home Care Packages to our valued clients living in their own homes.
This is a permanent Part-Time (4 days per week, with potential to expand to 5 days as our services grow).
Key Responsibilities:
- Deliver high-quality nursing care tailored to each client’s care plan and medical directions.
- Conduct thorough nursing assessments and collaboratively develop care plans with clients and their families.
- Offer guidance and support to the client’s care team, ensuring exceptional care delivery.
- Uphold best practices, ensuring all services are provided with compassion, respect, and in accordance with our policies and legislative requirements.
- Engage in team meetings and contribute to continuous quality improvement processes
About You
You are passionate about clinical nursing and dedicated to supporting our community. You thrive in a field-based role, traveling across the Mornington Peninsula and South Eastern suburbs to deliver exceptional care.
- Current AHPRA nursing registration.
- Prior clinical experience in community nursing.
- Experience as a Unit Manager and/or Team Leader.
- Strong skills in clinical assessments, care planning, and client outcome evaluation.
- Ability to provide holistic and person-centred care.
- Enjoys working collaboratively and in a team.
- Collaborative team player with excellent organizational and time management skills.
- Proficient in computer applications.
- Prior clinical experience in acute care (desirable).
- Experience with education and training (desirable).
You will also need or be willing to obtain:
- Current First Aid and CPR certificate
- Working with Children Check
- Police Check
