Registered Nurse - Community Hybrid

In this autonomous role you will be working as part of our dedicated homecare team playing a key role in delivering Home Care Packages to our established client base living in their own homes.
This is a permanent part time role, 4 days per week with the opportunity to grow to 5 days a week as our services expand.
Key Responsibilities:
- Provide quality nursing care for individual clients in accordance with their care plans and/or medical practitioner’s directions.
- Undertake nursing assessments and develop care plans in collaboration with the individual client’s relatives/carers.
- Provide guidance and supervision to the client’s care team.
- Demonstrate a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with General Homecare policy, procedures, and legislative requirements.
- Participate in meetings and contribute to quality review activities and processes.
About You
You have genuine care for supporting those in our communities with a passion for clinical nursing and working with clients to achieve their goals. You are happy to be in the field traveling to our clients home across the Mornington Peninsula and South Eastern suburbs.
- Current AHPRA nursing registration.
- Prior clinical experience in community nursing.
- Experience as a Unit Manager and/or Team Leader.
- Proficiency in clinical assessments, care planning, monitoring and evaluating client outcomes.
- Ability to provide holistic and person-centred care.
- Enjoys working collaboratively and in a team.
- Strong organizational and time management skills.
- Proficient computer skills.
- Prior clinical experience in acute care (desirable).
- Experience with education and training (desirable).
You will also need or be willing to obtain:
- Current First Aid and CPR certificate
- Working with Children Check
- Police Check
