  • Deliver clinical care in the home environment supporting clients to stay living in their home and connected to their community
  • Embody a person-centred approach, recognising and promoting the individual’s unique goals and preferences
  • Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
  • Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives
  • Effectively prioritise client needs in collaboration with other medical and health professionals as required

About You:

  • Your demonstrated compassion, empathy and advocacy. Our teams have a passion for what they do and the impact they make to our clients and their families.
  • Your excellent communication skills, both verbal and written to ensure the best possible client experience, teamwork and standard of care
  • Your ability to work independently and demonstrate sound decision-making
  • Your ability to work efficiently by optimising the use of technology within your role as well as prioritising your time effectively across your caseload
  • Current AHPRA registration (3 years minimum working as an RN)
  • Full Australian Working Rights
  • Current valid driver’s license and a licensed, comprehensively insured and road worthy motor vehicle
