- Deliver clinical care in the home environment supporting clients to stay living in their home and connected to their community
- Embody a person-centred approach, recognising and promoting the individual’s unique goals and preferences
- Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
- Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives
- Effectively prioritise client needs in collaboration with other medical and health professionals as required

About You:
- Your demonstrated compassion, empathy and advocacy. Our teams have a passion for what they do and the impact they make to our clients and their families.
- Your excellent communication skills, both verbal and written to ensure the best possible client experience, teamwork and standard of care
- Your ability to work independently and demonstrate sound decision-making
- Your ability to work efficiently by optimising the use of technology within your role as well as prioritising your time effectively across your caseload
- Current AHPRA registration (3 years minimum working as an RN)
- Full Australian Working Rights
- Current valid driver’s license and a licensed, comprehensively insured and road worthy motor vehicle

It is a mandatory requirement for a team member working in the Aged Care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role.
