Your Role as a Registered Nurse Chronic Disease

As a Registered Nurse Chronic Disease at DAHS, you will play a vital role in coordinating, managing, and promoting clinical management principles for individuals with chronic diseases. Your responsibilities will include but not be limited to:

  • Ensuring safe and efficient resource utilisation
  • Developing and implementing health programs for chronic disease management
  • Providing education to clients on disease prevention and treatment
  • Collaborating with internal and external stakeholders to deliver best practise client care

Your Profile

Our ideal candidate will possess:

  • Current registration with AHPRA as a Registered Nurse
  • At least 2 years of post-graduate chronic disease experience
  • Knowledge of Aboriginal health issues and community-controlled health organisations
  • Demonstrated competency in clinical management for chronic diseases
