Nurse Home Visitor

A Registered Nurse or Registered Midwife can apply for this role.

This role requires you to deliver the Australian Family Partnership Program (AFPP) to first time mothers in a home setting. AFPP is an evidence-based home visiting program with a focus on early emotional and physical health intervention aiming to improve the lives of vulnerable mothers pregnant with their first Aboriginal or Torres Strait Islander child. The program is voluntary for all eligible women and involves regular culturally appropriate home visits during pregnancy, infancy and until the child’s second birthday.

The Nurse Home Visitor works alongside a Family Partnership Worker to deliver the program content, building therapeutic relationships, stakeholder engagement and health promotion. All AFPP staff receive intensive training in their AFPP Home Visit Guidelines, screening tools and other materials to best support parents in improving pregnancy outcomes, improving child health and development, and supporting parents to develop a vision for their own futures.
