As the Nurse Unit Manager, you will:

  • Lead and work with a collaborative, supportive, and highly functioning interdisciplinary team;
  • Lead and develop exceptional clinical nursing practices;
  • Provide exceptional holistic care to older persons, with a strong focus on supporting the patient throughout their journey;
  • Manage the daily operations of the Bed Based Transition Care Program.

 

Classifications for this position will range from NM11 ($126,074 per annum pro rata)

About You

We are seeking a suitably qualified and experienced Registered Nurse with a background in Transition Care, Subacute or an equivalent setting. Your unwavering commitment to a person-centred approach to care will empower patients to achieve their health goals and enhance their overall well-being. Demonstrating collaborative leadership, a positive work attitude and excellence in practice, you will thrive on the unique and varied needs of our patient cohort, striving to provide the highest standard of care.

 

In addition to your experience, you will have:

  • Current Nursing Registration with AHPRA

  • A passion and dedication to providing exceptional care to older persons

  • Sound knowledge of the Aged Care sector and awareness of the policy environment underpinning the Transition Care Program

  • Demonstrated ability to lead, collaborate and engage with all members of the interdisciplinary team

  • Previous management or leadership /NUM/ ANUM experience

About the Bed Based Transition Care Program

The Transition Care Program (TCP) is jointly funded by the Commonwealth and State and Territories. St Vincent’s TCP is resourced to follow the client through the service and avoid unnecessary extension of hospital admissions and/or premature admission to residential care. The intent of the TCP is to provide short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.

The target group for Transition Care services at St Vincent’s are:

  • Older people exiting from hospital who, via participation in the program, achieve further improvements to their physical, cognitive and psychosocial functioning and an improved capacity for living independently; and/or

  • Older people exiting from hospital for whom the focus will be on actively maintaining their functioning while assisting them and their family/carers to make appropriate long term care arrangements.

The Bed Based Transition Care Program is located at St Georges Hospital, the Kew campus of St Vincent’s Hospital Melbourne (SVHM). Our nursing team is pivotal to the success of the broader, high performing interdisciplinary team, achieving ambitious outcomes while supporting the flow of patients throughout the broader organisation. We are a vital part of SVHM’s Subacute and Community Services, a key clinical program of SVHM and the broader St Vincent’s Health Australia (SVHA) Public Hospital division.
