Your Opportunity

Triple Care Farm Withdrawal offers a 10-bed residential program for young people aged 16-24, providing up to 28 days of integrated medical support, psychosocial rehabilitation, and brief treatment. The program includes six months of aftercare support, aiming to create a safe space for healing and wellness.

The goal is to empower young people to make informed choices and take control of their lives through comprehensive medical and holistic therapies.

In collaboration with the Registered Nurse (RN), play a pivotal role in supporting the nursing team within the dynamic withdrawal environment at Triple Care Farm (TCF). Engage actively with residents and healthcare professionals, ensuring seamless communication and strict adherence to protocols. Lead the charge in driving quality care initiatives for program participants and swiftly facilitate referrals for complex cases. Forge a strong partnership with the RN to closely monitor participant progress and adapt to evolving treatment needs, ensuring the highest standard of care.

Your key responsibilities will be to:

  • Provide comprehensive support to the nursing team in a withdrawal environment, collaborating closely with the Registered Nurse (RN).

  • Execute direct care activities according to nursing care plans, including assisting with resident care interventions.

  • Collect and maintain accurate healthcare documentation and records.

  • Communicate effectively with residents and healthcare team members, ensuring strict adherence to established protocols.

  • Drive and sustain quality care initiatives for residents participating in the withdrawal program at Triple Care Farm (TCF).

  • Coordinate referrals for severe and complex withdrawal cases to NSW Health partners when necessary.

  • Maintain close collaboration with the RN to monitor participant progress and address treatment concerns.

  • Offer a broad range of care and support interventions for young people with complex issues, including medical screenings and assessments.

  • Assist in the discharge process, including completing discharge summaries in coordination with the RN.

Requirements for Success

  • Current AHPRA registration as an Enrolled Nurse.

  • Experience in the Alcohol and Other Drugs (AOD) field desirable.

  • Proficiency in working within a harm minimisation framework.

  • Strong computer literacy.

  • Demonstrated personal effectiveness and ability to build effective teams.

  • Skilled in relationship management and client-focused interactions.

  • Keen organizational awareness and results-oriented approach.

  • Proven written and verbal communication skills.

  • Valid NSW Driver’s License and up-to-date First Aid Certificate.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
