Your Role:

As a Documentation Registered Nurse at St. Pauls Bene Aged Care, you will be at the forefront of our mission to provide exceptional care. Your responsibilities will include:

  • Comprehensive Assessments: Conduct and document initial and ongoing assessments of residents' health status, including medical history and current conditions.
  • Personalized Care Plans: Develop, document, and update individualized care plans in collaboration with multidisciplinary teams.
  • Daily Progress Notes: Maintain accurate daily notes on residents' conditions, activities, interventions, and any observed changes.
  • Funding Systems: Maintaining and conducting AN-ACC Assessments while working closely with both Senior Management and Allied Health Professionals
  • Medication Administration Records: Accurately record all medications administered, including dosages, times, and any effects or side effects observed.
  • Incident Documentation: Report and document any incidents or accidents, detailing circumstances and follow-up actions.
  • Effective Communication: Provide clear handover notes to ensure seamless shift transitions and document significant communications with residents and their families.
  • Legal Compliance: Record informed consent for treatments and ensure advanced directives are documented and respected.
  • Quality Improvement: Participate in documentation audits and reviews, contributing to continuous improvement in care quality.
  • Confidentiality: Maintain secure and confidential records, adhering to privacy laws and organizational policies.
  • Technology Proficiency: Utilize Electronic Health Records (EHR) and documentation software efficiently and accurately.

What We Seek:

  • Registered Nurse (RN) License: Current and valid registration.
  • Experience in Aged Care: Previous experience in aged care or a similar setting is preferred.
  • Comprehensive knowledge and experience of Funding systems: Demonstrated understanding and knowledge regarding the AN-ACC framework and proven experience in achieving AN-ACC targets
  • Strong Documentation Skills: Strong Attention to detail and proficiency in maintaining accurate and comprehensive records.
  • Excellent Communication: Demonstrated high level oral and written communication skills with the ability to be an effective leader and mentor of a multidisciplinary team.
  • Team Player: Ability to work collaboratively with a multidisciplinary team. Are able to β€˜get things done’, are a team player, available to lend a hand
  • Aged Care Passion: Experience in aged care with a deep commitment to improving the lives of residents, especially those with dementia.
  • Clinical Expertise: Strong clinical and assessment skills.
  • Leadership Qualities: Initiative, decision-making, and problem-solving abilities, hands on person, with high working standards; Are super organised, focused and methodical; able to multitask
  • Credentials: Bachelor of Nursing with current AHPRA registration, current National Police Certificate, and evidence of the right to work in Australia.
  • Health Compliance: Minimum 3 COVID-19 vaccinations and seasonal influenza vaccinations (recommended).
