Cardinal Stepinac Village is located in St John's Park in Sydney's southwest. We are blessed with a welcoming and home-like environment, emerging from the Australian Croatian Community and a true Not For Profit community organisation.

 

Cardinal Stepinac Village is a standalone aged care facility with a resident bed capacity of 144. As a residential aged care facility, we cater services tailored to High Care, Low Care, Respite Care and Self Care residents. We operate on a Nurse led, Case management model of care case managing each resident and always believe in following the evidence based best practice approach to care.

 

About the role:

 

We are looking for a passionate Dementia Nurse Specialist with a heart for aged care and expertise in dementia support to join our team on a permanent part time basis (37.5 hrs/wk).

As a Dementia Nurse Specialist, you'll play a vital role in providing specialised care and support to residents with cognitive impairment and dementia. We bank on your expertise and that it will make a meaningful impact on the lives of our residents.

 

The role of the Dementia Nurse Specialist is to assist in developing a dementia framework specific to CSV, in order to improve outcomes for residents and their families through leadership, clinical advice, consultation, education, and support of NUMs and other clinical team in the provision of evidence based complex aged care and dementia care.

The Dementia Nurse Specialist will triage referrals, perform comprehensive assessment, and participate in client case reviews in order to develop comprehensive behaviour support plans. Raise awareness about dementia and the advantages of early intervention via direct client/carer contact, improve access to information and resources about Dementia for people with dementia and their carers and families, and provide flexible support and advice to ensure the ability of carers to continue their primary caring role and families make informed choices about their loved one’s future.

Skills and experience

Essential:

  • Current APHRA Nurse registration.
  • Knowledge and experience of Assessment and Management of Behaviours and Psychological Symptoms associated with Dementia (BPSD).
  • Advanced clinical skills in dementia assessment, treatment, and evaluation.
  • Strong time management, organisation, and referral coordination abilities.
  • Excellent interpersonal and communication skills.
  • Exceptional assessment, judgement, and problem-solving capabilities.
  • Ability to work effectively in interdisciplinary teams and provide person-centered care.
  • Specialised knowledge in complex health, gerontology, and dementia care.
  • Experience delivering education and training to adult learners.
  • Proficiency in Microsoft Office and knowledge of relevant industry software.
  • Willing to work with the IPC and clinical teams and adapting to the changing mandates of aged care industry.

Desirable:   

  • Advantageous to speak Croatian language.
  • Flexible with the different shifts available.
