The Opportunity:

An exciting opportunity for an experienced and passionate Enrolled Nurse to join Anglican care Home Care, Community.  You will be responsible for assessing and coordinating care and support service to our consumers who still live their own home.

Along with the wider Home Care team, you will provide support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet the consumers' needs. 

The Care Coordinator role also incorporates providing clinical care within your scope of practice, which is based on your training and education, skills and experience.

What are we looking for? 

  • Managing individual care plans within own professional scope of practice or via appropriate delegation, referral and agreed reporting framework.
  • Demonstrating the highest level of collaborative communication across primary, nursing and allied health professionals to support an integrated approach to care delivery.
  • Provide clinical care within your scope of practice as required based on your training and education, skills and experience.
  • Work effectively with the Clinical, Allied Health and wider Community Aged Care Team to ensure the day-to-day operations are managed well
  • Identify individual consumer’s needs and implement a care plan and budget in consultation with the consumer, carer and relevant stakeholders (internal and external)
  • Ensure care plans are developed based on the goals, strengths and identified needs in consultation with consumers

 

About you?

  • Registration as an Enrolled Nurse
  • Current unrestricted drivers’ licence and a registered, comprehensively insured vehicle
  • A Current NDIS Check
  • National Police Check (Newcastle Anglican will consider the relevance of any reported outcomes)
  • Australia work rights
  • Sound knowledge and experience working with people who are ageing
  • Understanding of multidisciplinary teamwork and the ability to multi-task
  • Proven ability to conduct comprehensive assessments and develop goal orientated care plans
  • Computer and technology skills to enable use of contemporary systems
  • Skilled to engage and motivate consumers, families, staff and stakeholders
