Job Description

Location: Based in Campbelltown on the lands of the Dharawal People
Employment Type: Full time, maximum term until June 2025

Hourly Rate: $57.51 per hour

Are you an experienced nurse with a passion for chronic disease management and leadership? Join the My Care Partners Program in South Western Sydney as a Clinical Lead and take the lead in delivering individualised, person-centred care to patients with complex and chronic conditions. Your expertise will guide both patient care and the professional growth of the nurse care coordinators in your team.

About The Role: As the Clinical Lead, you will oversee the provision of holistic care to patients, helping them manage their health and remain well at home. You will use your clinical knowledge to lead, coach, and support your team of nurse care coordinators, ensuring the delivery of high-quality care that meets the needs of each patient. In addition to patient care, you will play a vital role in fostering shared care models, enhancing communication within the care team, and reducing preventable hospitalisations and emergency visits.

What You’ll Be Doing

  • Conduct comprehensive health assessments, recommending treatments and strategies for patients to manage their health.
  • Lead chronic care interventions and care coordination for at-risk patients in the My Care Partners Program (MCP).
  • Provide clinical oversight and support to Registered Nurse Care Coordinators, offering coaching, supervision, and training.
  • Ensure the program’s compliance with clinical governance standards, operational frameworks, and quality improvement strategies.
  • Work closely with General Practitioners, practice nurses, and other health professionals to ensure seamless, team-based care for patients.
  • Contribute to reducing hospital admissions and improving patient outcomes through targeted chronic disease management.
  • Support patients in setting health goals, developing care plans, and accessing appropriate services and supports.
  • Promote inclusive care that caters to the needs of diverse communities, including LGBTIQA+, Culturally and Linguistically Diverse Communities (CALD), and Aboriginal and Torres Strait Islander people.

Qualifications

What You’ll Bring

  • Qualifications: Bachelor of Applied Science (Nursing) or equivalent, with current AHPRA registration as a Division 1 Registered Nurse.
  • Experience: Minimum of 4 years’ experience in chronic disease management and a strong background in clinical leadership and staff development.
  • Skills: Advanced clinical skills in assessments, treatments, and patient education for chronic disease management. Strong leadership, communication, and team-building abilities.
  • Diversity Commitment: Experience working with people from diverse backgrounds, and a commitment to delivering inclusive care.
